We are now making appointments for flu vaccinations for October. If you are aged 65 or over or you have a chronic disease please ask at reception.

Children who are aged 2,3,or 4 years as at 1st September are also encouraged to have a flu vaccination (nasal drops) - please ask at Reception

We are also vaccinating for rotavirus for new babies. Please contact the surgery for an appointment.

If you live within our practice area you may register with us by either presenting your medical card FB4 (Fb4 New Born Baby). If you do not have a medical card please ask reception for a registration form. These forms are also available to download from the above "Registering Online" tab.

As a guide, the practice boundary, based on Ordnance Survey data, includes Gipton, and Oakwood to the east, Gipton Wood and Potternewton to the north and Sheepscar and Harehills to the south

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
